Required documents
To accept a participant, NDIS providers typically require the following documents and information to ensure compliance with the NDIS framework and to tailor support services appropriately:
- Proof of NDIS Plan or Access A copy of the participant's NDIS plan or approval letter. Details of approved funding categories and allocated budgets.
- Identification Documents A valid form of ID (e.g., passport, driver's license, or birth certificate). NDIS participant number.
- Medical or Support Documents Any medical reports or assessments relevant to the participant's needs (e.g., functional capacity assessments, OT reports, or healthcare provider notes). Therapy or care plans, if applicable.
- Service Agreement A signed service agreement outlining the roles, responsibilities, and terms of the arrangement between the participant and the provider.
- Consent Forms Consent for the provider to collect, store, and use the participant's personal information in line with NDIS regulations.
- Emergency Contact Information Details of a next of kin or emergency contact.
- Special Requirements or Preferences. Any specific cultural, dietary, or care preferences that should be considered in the service delivery.
- Optional (Based on Provider's Requirements) Support coordinator's details (if applicable). Bank details for payments if the participant is self-managing their plan.
